LVN Hospice Patient Care Planner (Intake Department)
The LVN Hospice Patient Care Planner is responsible for the intake and referral of Hospice patients into the organization. Accepts other referrals from patients, families, hospitals, physicians and other referral services when requested. Provides information about services and facilities. Develops and maintains rapport with members of the medical community to identify ways of best caring for patients. Coordinates with companies (i.e. durable medical equipment companies, infusion pharmacies, etc.) to ensure necessary equipments/IV medications are ordered and delivered on time.
Education/Experience/Skills/Qualifications
Graduation from a school of nursing accredited by the National League for Nursing. Possession of a license to practice as a Licensed Vocational Nurse in the State of California. Two years of experience in a health related setting/field within the last three years preferred. Knowledge of community resources.
